Modification to Opencart product display

Looking for a quote to modify Opencart 1.5.1.3.

Create the ability in the admin to allocate the following function to a product or not.

Ordering a particular product requires additional information from the shopper for each item. This requires 4 fields to be completed per item - for example if the quantity selected was '5' then 5 rows of 4 fields will be displayed and all mandatory. Please note that typical quantity can range from 1 to 30 items (so will require 30 rows of fields).

If you can suggest easier or better ways to support this type of data gathering then please let me know? For example, would it be easier/possible to support the additional fields within a new smaller browser window to open and display the rows of fields? Once the shopper completes and select "Done" it closes and saves the data as a Excel document attached to the order?
