Implement several payment APIs and deposit form

Description of requirements/features: I have attached an image of an example.

What I need to be done is to have implementations of 5 payment gateways on a simple HTML page, which I can later integrate into my own website. I do not want to use a cart system for this. (see image for example).

All you need to do is send them to the processing page, and then create a page that confirms the payment and then calls a function (that part I can write).

I need the following payment systems (APIs linked)

I use mysql for my websites, and I will design the interface but if you put in the placeholders (radio buttons and submit button) I will do the rest.
