I need someone to add a application form to my website

General information for the website: We sell wholesale
Kind of development: Customization of existing website
Description of every page/module: 1. I need someone to code/add a customer application form to my website like below;

This form will need to be filled out completely and sent to me to approve their credentials before they can login and view prices on my wholesale website. If I non-registered member visits my site they will not be able to see prices without approval of registration. I want it to say 'login for price' below product images.

2. I will also require someone to integrate an order status + shipping tracking like below;
http://www.artboxjewel.com/tracking/. I have no idea how this will work, but I want my customers to have these options and I need someone capable of explaining to me how this works and can implement it on my existing site.

I will require these to be added to my shopify store.
