Description of requirements/features: We are looking to add a gravity form (or any other suitable form) product add-ons to a pop-up box on the Product page of certain medications which have a restricted sale, rather than directly on the Product page itself.

We'd like the customer to add the item into their cart as quickly as possible. The customer will click "add to cart" and then see the pop-up box that will have the extra pop-up forms product add-ons for certain class of medicines before proceeding with sale.

This should be accomplished with the use of AJAX and some good custom code.

This is using any suitable plugin form; Gravity forms, ninja forms, contact form etc
