PHP/Mysql code to generate simple screen output

I require a php script that will query 1 mysql table and output the result to the screen in a certain format.

The code is used to query at a 1 table (page) that holds the title of each page of a web site. Each page will have child pages linked via a parent ID and the code will output the structrure that i will use to build a dynamic site map (Staring from the home page).
