Online form supported by a database

We are a small production/events company and I want to automate our 'Call Sheet' forms for our freelance crew members.

I would like to create an online interface which then creates a PDF of the attached document, and then it emails out to the various crew members detailed within it.

There would be a database driving it so that staff/job titles were listed there, with their mobile and email populating the form straight away as the name is selected.

Each field could be added to as the database grew.

Previous address for locations/contacts etc would build up in the background as more were added.

I'll look at designing a slightly prettier form but please see the attached to give you an idea of the kind of data and layout I'd be looking for.
