Need a MySQL backend setup to receive and report on data from CoffeeCup Form Builder

Database management system (DBMS): MySQL
Description of requirements/functionality: We have a series of simple Excel SS that are simply used for data entry (no calcs, just a few columns)
We are in the processes of re-cereating these docs as web forms using CoffeeCup Web Form Builder which has the ability to submit forms results to a MYSQL DB
We need this DB to be built as well as an admin front end that can view the submitted data in tabular form as well as report format
Reports should also be able to be printed.
The DB can be hosted on either our website platform (running Cpnael) or on a NAS at our head office
We also wish to be able to add new forms as required and have the DB manage them as well.
CMS and Admin requirements: Admin should have a desktop front end for viewing data and generating reports
Specific technologies required: Mysql
Extra notes: Must look pro and be able to easily scale as more forms are used to add data
