Filemaker application to run on Mac and iPad

I require a bespoke Filemaker application to run on Mac and iPad for managing and performing a multi-dimensional questionnaire / diagnostic. The app (for which a detailed requirements document will be available on Monday 23rd December) will permit:

- Setting up the core questionnaire
- Adding client specific questions
- Hiding certain sections / questions from any specific client
- Drilling down to a depth of 3 levels (i.e. follow on questions to be presented based on responses)
- Scoring the responses
- Associating one or more desired characteristics with each question / group of questions
- Storing client responses with a field for notes against each response

Ideally, the application will be delivered for acceptance testing on or before 14th January, 2013. Please indicate in your response if this is possible.
