Experienced Developer to setup a simple Database

We are looking for an experienced developer to setup a simple database and add some simple database functionality to a HTML website we are developing for a children’s charity. This small but wonderful charity pairs holiday-home owners who have signed up to offer a free week in their property with families who are desperately in need of a holiday or break due to serious illness.

It would be most useful if you can provide us with more information about yourself, experience and example work/websites and the best rate you can offer (please bearing in mind this work is for a very worthwhile charity)

We will send you a more detailed project specification on request but basically we need help creating and installing a MYSQL database on our web server, and incorporating it with our pre-designed HTML pages. The database structure has already been defined and consists of 5 simple tables.

The web database functions are as follows:

A password protected Admin area where the following tables can be created and maintained:
Patients / Bookings
Nurses
Holiday Homes
Testimonials
Supporters

A simple property search screen:
Accepts prompts for Area and Month
Displays available properties for the required area and months, with photograph and holiday home information from the database
“Booking Request” button that simply emails Admin and the holiday home owner
(The actual booking is made offline and once confirmed the booking detail is manually entered in the Admin area)
