Document Generator

I need a document generator that will create a legal form for me. The fields of the form can likely be drawn from a public database. That database is in MS Access. From this database with a little experimentation I believe an excel spreadsheet can be generated and from the spreadsheet a form generation system. Unfortunately I know very little about databases, spreadsheets, or document assembly so I must rely upon guidance from the seller. Here is the link to the public database:

Attached also is a xls spreadsheet that was created for me by another provider who obtained it through the MS Access database. He was able to assemble the parts I need from the database with some quiery work but he proved to be unreliable:

Last is a PDF form that will be the model form that must be autogenerated from the quiery, cullling, and parsing function suggested above.

I know this is a simple project for a person with skills.
