Database input form

Description of requirements/features: I need a database input form created to enable me to easily add entries to a MySql database. I have attached an example of a previous database input form I had created for a project a few years ago. I would like something similar but maybe more up to date. There was a problem with the text editor on that one creating weird code and not aligning text properly.

I need the input form to have password protected access so that nobody else can access or corrupt the database.

There are going to be a total of 18 small databases hosted on the website that mobile apps will source data from.

Ideally I would like one data input form where there is a drop down menu at the top that lets me select which database first and then I can input information. The information fields for all the databases will be the same.

I would like a Fixed Price for this job and ideally an example of a similar solution you have previously created for another client. I have no idea regarding an appropriate price so have set it at £50 min as PPH require a minimum and I will see what bids are submitted. I will not necessarily opt for the lowest bid as I believe in paying for quality work.
