Database format mysql lite

have a Moodle database module ( https://moodle.org/plugins/mod_dataplus) which I have used to create a questionnaire. I am looking for someone to assist me with design/formatting so that instead of scrolling down to answer questions the pages can be separated using tabs. I'm using Moodle 3.2 currently.

I would like to be able to export individual pages of report or export as a whole document (export to .csv and pdf).
I would like to set review dates and reminder notifications to be sent when due dates are approaching. Also when report is late.

Is this something you can assist with?

Do you have a dev site you can work from? How long do you estimate this taking?
