Create web form with multiple line entries that populate a database

We have a website: sjgolfballdrop.com that has a form that collects info about purchases for a fundraising event for our school. We would like the form to allow for 20 or so entries to be input on the same page. The student first and last name would be the same for each page. the other fields would be input and a separate record written for set of fields. The student first and last name would be added to each record in the DB so that the records could be sorted and/or exported to an excel file. see example form attachment.
