Bespoke application needed

I need an application developed for my students to supply evidence to pass their qualification. The students have to use photos, videos and other media to meet the standards of their qualification. The app also needs to have a chart so when they meet a standard, it reflects in a bar chart hat easily visible for verification. I have a non working word format so you'll be able to see exactly what functionality I need in the app. I would prefer someone to be able to communicate freely.
