Eletronic report card

It is difficult to know what catogery to put this in , so I will try a few, this won't be about web web-design, I currently have a 6 folded driving school report card, many times, my pupils misplace their physical progress report card. I would like to know if you are able to use my existing report card or design one so that I can mark my pupils progress electronically so that my pupils can see it but they cannot make any changes, I would like the facility to tick in boxes, I was looking for the correct terminology, I think the best way to describe it is AUTOCRAT? I'm not sure regarding the ins and out of doing this or the cost, cost,but if your quote is reasonable then please get in touch.
