Wordpress website enquiry form - if possible linked to smart sheet

I would like to remove current booking form on homepage and side pop out as these do not work on iPad.

I am looking to have a new Booking Enquiry form created - in the left sidebar where current booking form is.

The form needs to work across all browsers, mobile, iPad (http://www.ibizatransitexpress.com/contact/ - shows our current form styling)
