Wordpress plugin to process application

i need a custom plugin developed that will do the following:

users visit the site and take a quiz in a format like survey monkey. at the end of the quiz the user uploads some details (personal details and files).

When the user submits they are assigned a password in their email where they can login to view the status of their application

the users quiz answers along with their application details can be reviewed at the backend by an admin

the admin can set the status of their application.
