Wordpress form, NINJAFORMS , GRAVITYforms

I require a form designed and implemented into my existing website, if you have used NINJA FORMS OR GRAVITY FORMS or any other software /PLUGIN that you can use or feel is better suited to my needs

i all content ready and available , you just have to make it visual and the look and feel must be as required. once you complete the first design I will revise and provide details of a revision to finish as needed - this will not be completed until I see the end product as required.

this is not a complex job, you just need to be able to design as needed - I have all questions and text content ready for copy and paste. this is a quick job so I will only consider your hourly rate and you can tell me how quick you can complete this.
