Use Gravity Forms to set up an invoice

Num. of web pages/modules: 1
Description of every page/module: This is a simple job for someone who works with Gravity forms.
This is NOT a coding job.
I have Gravity Forms and Gravity PDF plugin set up.
What I need is someone to construct a form that takes:
The Name (First and Last) of the person filling in the form
The name of the recipient if different from above
Email of the person filling in the form
Email of the recipient if different from above
Phone number
Position of the recipient
Organisation / Company name
Address of Organisation / company including country and zip code / postcode
And a radio option for the package they want see: http://www.oxford-review.com/get-the-oxford-review-today/#
and turns it into a nicely laid out PDF invoice with my companies details on it (including Logo and bank payment instructions) so they can pay for a subscription through their company accounts and pay offline using a bank transfer.
3 things need to happen then
1. The PDF is displayed for download in the browser
2. It is also emailed to the client
3. A notification and copy of the invoice is emailed to me and cc'd to my bookkeeper.
You then need to brief me / provide instruction so that in the future when I change the packages I can do this.
Description of requirements/features: **ONLY bid for this if you have experience of Gravity forms (I am not paying for you to learn!).
I can't repeat enough - This is NOT a coding job. It is for someone to work within WP and Gravity forms and Gravity PDF.
Payment will be made after successful testing of the form.
