Opt-in page plug-in for Wordpress site

Kind of development: Customization of existing website
Description of requirements/features: I would like a plugin for an opt-in popup installed within my Wordpress site, this opt-in page/popup will be placed in the front page and capture the details of visitors. The details will then be forwarded to my email and I will then feed into mail-chimp or something similar.

I’m aware there are many plug-ins for this sort of thing to expect this project to be quick and cheap.
