Joomla - Chronoforms - multi page

Description of requirements/features: I have a form made in chronoforms that I need to replicate. The origional was done in chronoforms 1 on joomla 1.5 and I need to recreate it in chronoforms 5 on joomla 3.4

I need to recreate the quick price form at the top of this site, it starts off on the homepage, in a module position. But then on submit, it continues on a different page. I assume in either the component area or perhaps a plain module position.

So far I have created the form, but have only managed to get it to load the next page in the same position on the homepage.
