Implement a form that uses API to send for lead generation site.
Currently have the code working on a site but need different form fields on a new site.
API currently working on another site, so has been tried and tested.

What kind of request does the API expect? (GET, XML POST, etc.)
How many fields does the form have? Are they all text inputs?
