I use Authorize.net and need a payment site setup

I need a page setup to accept online payments. I currently have Authorize.net

1. The customer paying will enter their Loan Number and Last Name to login (I will provide csv of loan numbers and last names)
2. The customer will then use Authorize.net to submit a recurring (default) or one-time payment via check, debit card, or credit card.
3. Email them and myself a receipt for their transaction. (I need to be able to change the admin email address)
4. It also needs a simple contact form to take First Name, Last Name, Phone, Email and send it to admin.

I would like to use Joomla if possible, I plan on purchasing a template from templatemonster.com. If you can integrate the logo and do some customization, I can pay a little more.
