I need professional help with php, forms and databases. (not Wordpress)

I need someone to check the php I have done, so far, for the forms on a website I have built and to attach them to a database. I have two types of forms, one is a general contact form and there are several email only forms, on various pages, to simply collect emails. I have made databases for both.

I am in the US, specifically California, and the site is a music site. I'll need an estimate as to how long it will take to get everything working well, which will require that I give anyone doing this more details about the current state of the project.
