I need my geo directory plugin on word press customised slightly

Good morning, as the title suggests I need my plug in customised, it is a business directory/ review site, at the moment when someone searches for a company that isn't in the directory it returns a 'no listings found' message. What I want it to do is to state 'no listings found, be the first to review this business below'. Then below I want there to be a box that states 'essential info' and sections for; business name, category, business location (just town or city), the reviewer name, reviewer email address then the review (there is a review element to the plugin already so I'm hoping that you can just take the code already written for that and just enter it onto this page). After that a button that states post review or something similar. Underneath all that I then want there to be another section that states 'non essential information but really helpful' which has; business address line 1, business address line 2, business postcode, business email address, business phone number and business website. Then another button below that (exactly the same as the first button saying post review). When either button is clicked I then want a listing to be created with all the information from this page, I have a screen shot of how I would like it to look on my pc but I'm currently on my phone so will upload that when I have a chance, please can you give me a quote?

Screen shots now uploaded in the form of an excel spreadsheet that i have put together, this is how i would like my site to look, in relation to a business that is already listed, i would ideally like it to go straight to the review section of their profile when the user is on the 'rate a tradesperson' page instead of the businesses 'profile' section. However this isnt essential, but would be great if its possible to do this as well. Also just to clarify when a user searches for a business and it is not listed, i don't them to have to 'log in' or 'register' to create the listing and review.
