I need a Gravity Form fixed on my site

Description of requirements/features: I need the form on this page fixed: http://dev-omghome.pantheonsite.io/project-planner/

It's a Wordpress site running Gravity Forms with conditional logic (fields will appear based on what the user chooses in the first dropdown field of the form). One day it just stopped working and I'm not sure what's wrong. The forms without conditional logic display fine on the site, but this one doesn't. From my research I believe the problem is the page is loading multiple versions of jQuery.

Please note that this is a dev site, so any changes you make will either have to be applied to the live site, or you have to tell me what you did to fix the problem so I can apply it to the live site.

One final note: Please don't try to fix it by forcing the element to display with css, other people have tried that and it's not what I want because it makes all fields display all of the time, and I want the form to retain the conditional logic.
