I need a fully responsive 5 page Worpdress website that connects with Leadsius

General information for the website: I have chosen the Worpress theme which is free. I also have screen grabs, website examples and a clear spec for requirement.
Kind of development: New website from scratch
Num. of web pages/modules: 5 pages
Description of requirements/features: I have the domain name already and know where I want the site to be hosted. The Home page will have a 2-field form including dropdown menu and email address fields over a large image - I will upload images at a later date. Only the Blog, About us and contact page will be accessible from the home page. Blog page with email sign-up. Contact page with contact form. About us page with contact form. One top level page with changeable H1, H2 and body text. I have a Leadsius account and will provide you with tracking code. The forms should come from Leadsius. I will also proved Google Analytics code to be added to site.
