Help needed to create a form

We are going to build a new site based on double glazing windows and we are trying to replicate what this site has done http://www.iwantawindow.com/measure.

If you type a typical window size in the width (W) and height (H) boxes e.g 1800 x 1200 it then takes you to this page http://www.iwantawindow.com/windows where it shows you the available styles and greys out the one unavailable and shows the price in the bottom right hand corner.
