Develop a new page on an existing wordpress site to collect candidate data

This job is for an experienced Wordpress developer who can design 2 web pages on an existing website to collect and search simple data.

I am looking for a fixed price proposal to do this work please, but as I dont do this type of development I have not set a budget for it. Payment in full on completion via this website.

PAGE 1:

I would like to add a new page to our site at mercianlabels DOT com/careers/ (WordPress 3.4.2) to enable website visitors who are interested in working for us to leave their details on a database which we could search when we have a vacancy to contact people with a suitable background.

Fields needed to be collected:

Data input date
First name
Surname
Permanent private email
Permanent private phone number
Home town
Postcode
Linkedin profile URL
CV upload and storage with a max file of say 5MB
Roles sought : List of roles to be included (that we can add to or amend easily using the interface you design)are product innovation specialists, managers in any relevant field, production supervisors, internal sales coordinators, field key account managers, label printers, dispatchers, open source IT staff, graphic designers and internal accounts / credit control staff.

If possible, and email notification to us when a new entry is made.


PAGE 2:

The second part of the job is to design a second (password controlled) page for use of our authorised staff only to view all the information posted easily, and enable us to search for all candidates with certain skills by selection of dropdown role AND all fields keyword search sorted by reverse date.

No specific time limit on the job, but I would expect it done within a couple of weeks from instruction.
