Customize and integrate WordPress Store Locator Plugin and Forms Plugin

I am looking for creating a hospital locator which will look and function similar to https://myclinicals.acmedical.org, in which they have used THE NAVIGATOR plugin with the GRAVITY FORMS forms plugin.

I am thinking of using the AGILE STORE LOCATOR plugin ( https://codecanyon.net/item/agile-store-locator-google-maps-for-wordpress/16973546) and the ULTIMATE MEMBER plugin (https://ultimatemember.com/features/)

The objective is:

1) As seen in the https://myclinicals.acmedical.org when a new user goes to that URL, then the user will see the Map with List on the right (using the store locator plugin) and a prompt window should ask the user" Rotation City/State" > followed by prompts for similar information from the user as seen in the https://myclinicals.acmedical.org while user tries to access the hospital locator.

2) After the required information is entered by the user, the user should get access to the map in the store locator with the selected state zoomed in for his/her view with lists of the Preceptors/Doctors on the right side as seen in the example. I would be adding OTP/2FA using MINI ORANGE (https://wordpress.org/plugins/miniorange-otp-verification/), which needs to be added to the prompt to verify the user when the user enters the mobile number in prompt window.

3) In the example https://myclinicals.acmedical.org, the pins on the map is a rectangle with price on it. But in the proposed new one, It would show Preceptor Name/Number. When the user clicks on the pin, then a pop-up window should appear and show similar information shown in the example https://myclinicals.acmedical.org

4) Please note that in the example https://myclinicals.acmedical.org the first time user can close the prompt window and access the stor locator by clicking the (x) close button on the right side of the prompt window. But in the proposed one the new user should not be able to close/skip the prompt window.

I would be buying the paid plugins, and i am inviting developers to customize and integrate the same to achieve the above mentioned functionality.
