Customise Formidable Forms Summary view

Hi

**Please apply only if you have experience with Formidable Forms plugin ONLY**

I am looking at someone with expertise in Formidable Forms (Wordpress Plugin) to assist me with customising a summary page.
I have a multi-page product order form created and the last page (summary), I just need present a different layout view of the summary.
Please see attachment - intrinsic-extrinsic-notrequired.png where most radio button has selection of `Intrinsic`,Extrinsic`, or `Not Required`.
summary.png shows how the view should be if user made selection.
