Customise a form in wordpress and import data from existing MS SQL database

We have a new website development project for a recruitment firm. We have already customised a theme and almost created all the pages. Their existing website (that is in ASP.NET) has a form via which the visitors registers to their job applicants database. They have a current database of 65000 registered users which needs to be imported in our new website. The data has to be imported with labels attached to them which will be later managed in WP form backend.

Time is essence. We would like to get this done in next 10-15 days.
