Create form with calculated fileds plugin of elementor

I need an order form where I have
Description product 1 / Price / Number (this field to fill in) / Total of this line (calculated)
And this for several products (+- 20)
Last line is shipping which is 2,00 for 1 product or 0,00 if more products (so calculated field)
At the bottom total must be calculated
And name address needs to be filled in.
Then this is submitted and all info is send to e-mail address.
