Cost estimation form to be built

I need to build out a Cost estimation form using the following software:

I have purchased the licence and can provide an example to base the design on.

There will also be cost variants involved such as, Price is dependant on number of users.

This is outside of my skillset and would take a long time for me to produce myself, please get in touch if you have experience using this software and feel confident about completing the job quickly.
