Caldera Form - Wordpress Application - Create form based on word document.

Hi.

I have a quick job - create an application form in the Wordpress on Caldera Form.
It's about 16 pages in word, however, there are many big fields for notes.
In many situations it's just question - yes / no.
Attached word file and PDF how it was generated, but some unused and unnecessary fields were removed.

The form needs to be created in Caldera Form, which can then be imported to my site.

Deadline to finish is Thursday night. I need it on Friday morning.

Additionally - I need HTML file which will look similar to the WORD / PDF file, with tables with questions and values for additional output.

I will answer to all questions asap as need to choose freelancer tonight.
