Add a feature on a WP theme

I need to add another icon which calls a form, under the Feedback/mail button mentioned in this theme: http://demo.themedev.me/wordpress_themes/hq/

Look at the far right and you see 3 icons. I want it below the 3rd one, as a fourth icon. The form (a simple booking form) is ready from another site but I need that implementation.

The developer who will do this will also need deliver instructions and alterations for this work.
