Survey that I can analyse results

Hi I am looking to create a survey that will give a final score for the user, The score is calculated depending on what answer they give. For example if the person scores 1-2 indicates there are in group 1 3-4 indicates there are in group 2 5-6 indicates there are in group 3 7+ indicates there are in group 4 Q1. A-1 B-2 C-3 D-4 to the user it would look like Q1. A, B, C OR D Q2. A-1 B-2 C-3 D-4 Q3. A-1 B-2 C-3 D-4 I dont want the user to see the numbers This person answered A,.C & D which gives them a score of 8 7+ so indicates there are in group 4 Can we do a survey like this. Does that make sense> Thanks
