Someone to install a MySQL script for me

I have a MySQL database designed on PPH the orginal desinger is no longer active, ?I need someone that is familiar with MySQL and permissions to 1, setup the script to where i can access mobilecarcreditnow.com/admin so that I can setup the database/script and 2, tell me what you did to get it to work as I will be setting up more sites with this script. When I follow the instructions (see attached pdf) is the an error screen (see attached screenshot. It should take no more than an hour and will pay a min. of a hour regardless of if it takes 5 minutes or a hour. I will give the person I choose for the job an IP address an ftp username and password to work with
