Software/Web Development for a Kiosk at the Grand Pier Weston Super Mare

We have developed a feedback survey on a Kiosk which is at The Grand Pier Weston Super Mare. The problem we have is people are not leaving feedback. We have decided to change the Software on the Kiosk so it collects demographics instead. The customer is incentivised by entering into a prize.

We need the software written so that the Kiosk collects demographic information, the customer will then be emailed with a link to then fill the customer feedback survey.
