Small Micro website

I need a micro website with a submission form to capture my leads and email over to me. The site will basically be a couple of pictures, graphics, text and a nice clean and professional image with a form to capture lead information for us. Someone whom can help give it a bit of branding would be best as we can only provide wording at this moment. Ideally need couple of mock up designs initially.
