Slice 3 images to HTML and implement forms

I need 3 images sliced in HTML. You should first slice these images and them implement the images to HTML where's forms build with tables. These 3 images have quite same structure, but forms have different fields. The files where you should implement the new images and HTML have some php there. You should be able to read it a bit so you don't break anything, but you don't have to do any programming. The code is ready, only thing is to slice the images and implement the new layout to HTML.
