Simple J query quiz edit

I have a quiz that I have found and which needs editing to work in a particular way,

the quiz is to be embedded into a squarespace site which I can do myself, all i need are the bare bones of it so that all the functionality works in the way I need. all css etc I can apply myself.

The reason I like this quiz is that I know I can embed it into squarespace and equally all the css is there for me to edit so if you can edit this or have something that works in the same way that'd be great. So the quiz needs to work in the following way;

-10 multiple choice questions
- rather than in a list format I need each question to follow on from the next, so after question 1 there is a 'next link' (not a button) that takes you to question 2, the quiz should always remain in the same window.
-after question no 10 your results and the right answers will be displayed.
- if you have scored less that 5 questions right you will be asked to perform the quiz again - more than 5 and you will be provided a link to a download.

EXTRA FUNCTIONALITY

this is only really if able to be included in the budget would be facebook/ twitter sharing of your result however I am aware that this is getting a little complex so please advise accordingly.
