I need a database driven programme (maybe even using MS Access to store the data) to record operating notes.
The database should be able to reside on a central server, but allow the interface for data input to be opened on a number of computers on the intranet network at the same time.
There should be a use name/password for each individual needing to access the database and enter data (but not delete), and a master administrator username/password which will allow addition of more users, as well as editing/deleting of data.
The database should also record date/time/person who modified or made an entry.
The database will have the following fields:
1. Patient - first name(Free text box)
2. Patient - surname(Free text box)
3. Patient hospital number(Free text box)
4. Patient dob (date input in DD/MM/YYYY format)
5. Sex of patient - male/female (radio button)
6. Date of operation (date input in DD/MM/YYYY format)
7. Commencement time of operation (there should be a START button on the interface which will allow one to press it, and it automatically capture the date/time from the computer clock)
8. Finish time of operation (Same as above, but for finish time)
9. Name of operation(s) (can be drop down box with user selectable list) – this should then generate:
a) in one box, the automatic operation record with list of postoperative nursing instructions including follow-up arrangements which is then amended by surgeon as required within the box.
b) in another box, an automatic automatic GP discharge letter including follow-up arrangements which is then amended by surgeon as required
c) in a third box, an automatic list of medications to take home which is then amended by surgeon as required
** For (a), (b), and (c) - the text can be automatically obtained from a modifiable set of files ?txt/.doc or something similar.
10. Code(s) for operation(s) (free text box)
11. Right/left/bilateral (radio button)
12. Preop diagnosis (diagnoses) (free text box)
13. GA/LA/LA sedate (radio button)
14. Surgeon names (Free text box)
15. Assistant names (Free text box)
16. Scrub nurse names (Free text box)
17. Name of consultant patient registered under (Free text box)
18. Operating theatre number (Free text box)
19. Complication(s) Yes/No (radio button)
20. Free text entry for complication (if yes selected above).

The database must be updatable, mainly the source files for some of the text as mentioned above for (a), (b), (c), as well as user list and administrator list. It should allow searches in future on the source file for operations with complications / all of a particular operation type / all operations with complications, etc, easily.
That's probably almost all the details anyone will need.
Proposals and bidding welcome, and proof of sample database or similar work done will be beneficial.
Payments only through this websites system, and will be on an all or nothing policy.
Partially completed work / almost complete = No payment.
