Several files turned from individual .doc files to one large file , with mods

I have folders on 12 clients , each folder has several(10ish) .doc files in it. I am required to update each one monthly, the date and a comment box, i would like 1 file per client, that i can open each month will update the date on each section when i change on first page and then a comments box on each page i add comments to, old comments being stored on for reference, i use open office on mac currently, sample file attached of one of the .doc files attached,
