Reporting forms for NGO in Africa

We are an mission NGO in Africa (small) and we need a way for our staff to report on their activities via a web template. They would need login access, and there would need to be some 'forms' they fill and place photos / stories, attached spreadsheets or other documents. This will then need to be sent to the managers and supporters overseas or be viewed online. These reports would need to be stored on a server so that access can take place in future.

We would need 2 forms that can be edited if needed from the backend. Attached are some outlines of the forms.
