Redesign the payment portal

Oil Guru Recruitment takes payments for its CV writing services via a paypal gateway. The payment page needs to be redesigned in order to stream line the process and maximise income. I would like the page to be able to give the customer the choice of services in an interactive way. The current page can be found here: http://oilguru.org/premium-services/cv-writing-service/

I would like the following options on the new page:

1. Drop down menu offering Size of CV: 2 page, 3 page, 4 page, 5 page
2. Drop down men offering Deadline for service: 5 days, 3 days, 2 days
3. A tick box for editing and adding a profile image.
4. A tick box for adding a covering letter.
5. A upload file option so that the customer can upload their current CV.
6. To be able to edit the backend of the form with regards to price, text, payment account, ie PayPal and also be able to add additional services.
7. To enter email addresses that get notified when an order has been processed.
