Quote me on K2 extentions extra feature for Joomla

I'm looking for someone to create an extentions for Joomla K2 that does the following:

K2 will be used to create a product catalog for a customer. Products will be items in different categories with different "extra fields" on which a user can filter to find the product of his choice.

In the overview, the user will see all items in the selected category. If he clicks on "read more" for a specific item, he will get the detail information for that single item.

All the above will be installed and configured by us. What we need is someone who can create an extention that creates a button on every single item view page to request more information.

When clicking on this button, a form should appear that allows the user to request more information.

This form should contain:
Name:
First name:
Telephone:
Email:
Subject: (=Request for information "product name")
Message:


-> Who can help me with this at what price? If you know a ready made extention for this, you can use that one as well for this purpose.
