Purchase of a domain, Install a wordpress theme, slightly edit the theme and host the site for 12 months.

Hi, I need a domain to be purchased along with hosting for at least a year. I’d like a word press theme installed and slightly customised for the running of an affiliate site. The Job needs to include a logo, slightly customised word press theme, one email address and I would be looking for continued support should I need to tweak anything (Nothing major once the design is in place). If I am happy with the work, I will be looking for 2-4 more sites of a similar set up over the next few coming weeks.

I’m sure it is a wordpress theme but I do not know which one? The design will be very similar to this one, I have a design knocked up in paint which I can email to you to clarify the design template.
