PPC Website with DW CS3 template

Hi,

The details are a little long but figured too much information would be better than too little for accurate bids.

I want a website built for Google PPC campaign, the site is going to be about 10 pages but I will be updating most of the pages myself once the template and a few key pages have been done. I started it myself but simply haven't got the time or the knowledge to complete the site the way I want it, this is my effort!

I would like each page to have an enquiry form on it (except the contact us page, which will just have a simple contact form), client and server side validation for the forms obviously.

The quick enquiry form can go either at the top of the main content or in the right side bar, enquiry form to include these fields:

First name, Surname, Phone No, email, loan required, loan term (drop down with 3 months, 6 months, 12 months, 24 months, 36 months, 48 months), Purpose of loan (Residential or commercial (buttons)), Adverse credit (buttons: yes / no), additional information (small text area) also a gentle call to action on form and submit button (not just submit)

Depending on costs, I ideally would also want a full online application form on a separate page to include everything on the enquiry form above plus fields for a 2nd applicant (name etc), Previous/maiden name, Date of birth, address, security address (if different from address), estimated Security Value, Security Description (radio buttons: Property, Development, Land), Additional Information (text Area) - If this is within my budget I would also want a link (image) somewhere on the header or side bar promoting the full online enquiry


Also want a simple loan calculator on the calculator page, once users have entered details on the loan calculator, a slide down will ask them if they want to apply for the loan, similar to this http://www.bakerfinancial.co.uk/loan-calculator.html, can give the calculation for this if needed.


Site needs to be modern with a clean / crisp look and feel to it. The enquiry form and calculators obviously need to be visually appealing as well.

If possible would also like some form of flash / animation on the page to incorporate call to action / benefits, so it looks more appealing (not just text) - this can go below the top nav bar stretching the width of the site or wherever as long as it looks right.


To summarise: I need the Dreamweaver CS3 template, an index page with enquiry form as above, contact us page with simple contacts form and calculator page, and menu (navigation) which I can alter myself if necessary (not gif images for the links)
I would require all .psd files etc upon completion of the work.
With this being a PPC site, it must be visually appealing, clean and modern.

If you can better the logo I have done then we can discuss a price for that also.

Happy with an hourly rate as long as an (accurate) estimated timescale can be agreed, I would be unwilling to pay more than £100 upfront.
