PHP Script create MYSQL SELECT from HTML form + output results

I have a database and I need to create a html form with php to search the database.

I want to search on the following criteria;

'description' using text box
'platform' using drop down list
'type' using mulitple check box

The search needs to be able to search for any combination in the form. Also, i need the option of 'any' for the drop down list and check box options.

Please note, the check box relates directly to the items in 'type'. the database has the following items in the 'type' column;

Local_exploit
Remote_exploit
web_exploit
Shell_Code
dos_exploit
White_papers

Search example might include 1 or more 'type' items or it could be 'any' type.

I am posting this job becuase I am struggling with how to make the php contruct the SELECT statement when multiple checkboxes are enabled.

I have attached the database to work with. do not worry, its a public database so no personal or confidential data is included. the DB comes from www.exploit-db.com, you can see their search form in action, i would like the same, only simplified to the above described HTML form.

I also need a php script for the results of the select statement to show the output.
